“my MedBio” is your personal health information record.  "my MedBio" is a concisely organized record of your medical history that contains important health events, conditions, diagnostics and medications that would be essential information to health care providers that may need to begin an immediate and critical course of treatment to you.  “my MedBio can speak for you if you are unable to communicate or can not remember all important details.  "myMedBio" also contains the personal information that you deem as very important to anyone that may be providing treatment to you.  “my MedBio” includes a hard-copy (paper) version and a pocket-sized CD that is easy to carry with you.
 

Why have “my MedBio”?
Your health care is a shared responsibility between you and your health care provider.  During a health care encounter, your personal health information can be a critical determining factor to the nature of treatment that must begin.  Your provider many times only has access to what you give them.  Since your complete personal health information may be spread through many different physicians and facilities, it is absolutely necessary to have your medical record organized in one place.  Subsequently during an encounter with medical personnel, we inevitably may forget some significant piece of information or may not be able to communicate at all.

Where do I keep “my MedBio”?
The hard copy should be kept in a conspicuous place within your home.  Some examples may be places such as, next to the main telephone or stuck on the refrigerator.  This copy should be placed in a location that could be easily picked up and taken with you even in the event of an emergency.  The business card sized CD version can be easily stored in your purse or on your person to have with you at all times.

How does the CD version work?
 The CD version can be inserted into the CD drive of any computer.  Everything from that point is automatic.  The CD contains a program that automatically reads the CD and displays the exact same information and look of the paper copy that can be easily read or printed.

How Do I Get "my MedBio" Started?
Simply give us a call at 980-0700.  The process of information collection is typically begun through a personal interview.  We will arrange an appointment with a member of our staff to begin the process.

What else can be included on “my MedBio”?
“my MedBio” is a perfect place to keep Advanced Medical Directives.  These documents allow you to name someone to speak for you in the event that you can not speak for yourself during a major medical event.  Further these documents can guide your physician in critical treatment decisions according to your wishes.

Can I give a “my MedBio” as a gift?
Yes, “my MedBio” makes a great gift for your loved ones.  Gift Certificates are available by calling our office at 980-0700.  “my MedBio” can be a gift of security and peace of mind for both the recipient and the giver.

Can I get extra copies of “my MedBio”?
Yes.  Many times another family member that is closely involved in the health care decisions may want to keep an extra copy.

Can “my MedBio” be updated?
Yes.  Keeping “my MedBio” up to date with necessary information is very important.  You will have the option of updating as needed or doing periodic reviews.
